Full job descriptionAbout the roleThe purpose of this position within the Residential Transition Care Program is to deliver Social Work clinical services, in accordance with established clinical protocols, prescribed professional standards and the Queensland Health Code of Conduct.The position provides an opportunity for a new graduate Social Worker (applicant to have completed their Social Work qualifications within 12 months of the closing date).

Provide case management services, including liaison with patients and their families, other health professionals, supervisors, students and researchers.
